Topical immunomodulatory drugs

For children who do not respond to mild steroid ointments or who need to use steroids for a long time, it is necessary to consider using topical immunomodulatory drugs. Elidel (pimecrolimus) and Protopic (tacrolimus) are two products that can be used for children.

(1) Topical hormone ointment

Appropriate topical use of steroid ointments is safe and the most basic treatment. Ointments, such as petrolatum, are gray and thick. Creams, on the other hand, are white and contain more water. They must be applied once or twice a day to the red, inflamed areas. This is especially important after a bath, when the skin is still damp. The steroid ointment should be applied first, and the moisturizer should be applied to the skin areas without redness. Moisturizers should not be applied before steroids. Mild topical steroid ointments such as 1% hydrocortisone, triamcinolone, or hydrocortisone-17-valerate ointment are effective enough for most children. Occasionally, your doctor may need to use a stronger steroid ointment.

(2) Antihistamines

Taking drugs such as diphenhydramine, hydroxyzine hydrochloride, and zoltec (cetirizine) 30 to 60 minutes before bedtime can help children have a comfortable night and sleep well. Some children may become more irritable and angry after taking histamine. You must tell your doctor and stop using the drug.
