1. Symptoms: The shunt flow is small and often asymptomatic.

The shunt volume is large. Since the aortic pressure is higher than the pulmonary artery pressure, there is a continuous left-to-right shunt in the whole cardiac cycle, the pulmonary circulation blood flow increases, the pulmonary artery and its branches expand, and the blood flow returning to the left atrium and left ventricle also increases accordingly. The diastolic load of the left atrium and ventricle increases, and the ascending aorta expands. The size of the shunt volume depends on the thickness of the unclosed ductus arteriosus cavity and the pressure gradient between the aorta and the pulmonary artery. A large amount of left-to-right shunt can cause pulmonary hypertension. In the late stage, if there is obstructive pulmonary hypertension, the pulmonary artery pressure is close to or exceeds the aortic pressure, then the shunt is reduced, stopped or right-to-left shunt occurs, and right ventricular hypertrophy, cyanosis and clubbing of fingers (toes) occur. Because the shunt level is on the distal side of the left subclavian artery of the descending aorta, cyanosis is more obvious in the lower limbs. The left-to-right shunt is at the aortic level, the aortic diastolic pressure is reduced, and a series of peripheral vascular signs such as increased pulse pressure appear. .

2. Physical signs:

The apex beat is enhanced and shifted to the lower left, the boundary of cardiac dullness expands to the lower left, there is a loud continuous murmur at the lateral side of the second intercostal space on the left edge of the sternum, which is transmitted to the upper left back of the neck, accompanied by systolic or continuous fine tremor. After the occurrence of pulmonary hypertension, only the systolic murmur may be heard, the second sound of the pulmonary artery is hyperactive and split, and the pulmonary valve may have a diastolic murmur of relative insufficiency. When the shunt volume is large, due to the increased blood flow through the mitral valve orifice and the accelerated rate, there is a short mid-diastolic murmur at the apex. There may be peripheral vascular signs, including: enhanced carotid artery pulsation, increased pulse pressure, water-rush pulse, capillary pulsation, gunshot sound and Duchenne sign.
